Dying on teleport

Whenever I teleport I die. No error in console and I have added all version of air blocks. My spawn is not at 0, * , 0. Could this be the issue? There is no option to configurable where they teleport from/spawn point.

Where do you die and to what cause

I know I've had players die when they drop from the sky, disabling that seems to work. Not sure if it's related to the issue above.
