[16 hours] Query-able vs. Checkbox

Currently, the initial requested implementation is for a three-tiered approach with the final tier being a search input for available ports. For 30 or so ports, that seems a bit much - particularly if a user doesn't know what list of ports are available.

If not:

• revamp /templates/visualize/includes/vtr-cas.html template and use the logic similar to /templates/visualize/includes/catalog-layer-row.html
• Ensure that a user can select multiple ports without the checkbox closing out
