It would help to have a list of checkboxes guiding contributors along with filing a higher-quality bug report.

Items should contain:

"I'm using the latest vscode extension version"
mention vscode extension number
mention reproduction steps
mention expected behaviour
mention actual behaviour
include any related screenshots
include misc comments
