HDMI digital audio output device is not available on HA OS on RaspberryPi4. I also tried a fresh install of HA OS but the device didn't show up. For reference I installed a HA Supervised instance on Debian and there the HDMI output was available from the start. So I can rule out a hardware or cable issue.
I do get a video output on the HDMI port with the HA OS.

I just found this old thread where I found out that one of the “Build-In Audio Stereo” devices is the HDMI output. So this is more about changing the title of the device.
