Volunteer as Facilitator

Allow people to volunteer to be a facilitator through the app.

Note: this version would be for anybody filling out the form. #22 is a process for basically "opening" a club.

The facilitator would have to indicate how soon they can start, and then choose their times. They need to agree that they meet requirements. They might still need to be approved (basically put info in the google sheet and maybe I'll uncheck them).

There should be a formal approval process. When they're approved, that should trigger a post in the associated channel. I think doing that via Google Apps Script might make the most sense.
